Breast Cancer Education Project Grant
The Beth C. Tortolani Foundation is offering grants for existing or new programs with innovative projects regarding breast cancer meeting the following criteria:
• informational programs directed towards individuals diagnosed with breast cancer
• informational programs directed towards the families of individuals diagnosed with breast cancer
• informational support programs for the community referencing breast cancer
GRANT AWARD PERIOD: One year
APPLICATION DEADLINES: 2/1/12, 5/1/12, 8/15/12, 10/1/12
FUNDING: $1,000-$10,000

The Beth C. Tortolani Foundation is proud to have awarded grants to the following programs:
2012
Necessities, Inc. - "Necessities Bag Distribution on Long Island" Reusable tote designed for the pre and post mastectomy patient providing information and supplies to prepare for surgery and aid in recovery. Also included in the bag is a booklet entitled "The Woman To Woman Guide To Prepare For Mastectomy" written specifically for patients and their caregivers which is also available in Spanish. Bags are provided free of charge through physician's offices. Program Director: Fran Mendelowitz
Lean On Me Breast Cancer Network - "Peer Support Program" One-to-one emotional support is offered to newly diagnosed men and women with breast cancer from diagnosis through treatment and recovery in the privacy of their physician's office. Each patient in this program will receive a "Necessities Bag". Patients are also informed about the availability of receiving free wigs and post mastectomy bras and have access to free seminars, support groups, written resources, as well as the Lean On Me website. SHARE - "Young Women and Breast Cancer: The Empowerment Series" Monthly support group for women under 50 years of age in various stages of their cancer journey facilitated by two survivors. Topics include nutrition, fitness, fertility, family planning, caring for young children during treatment, career and dating issues. Program Director: Ilene Winkler
2011
New York Hospital Queens - "Breast Cancer Resource Room" This resource room for breast cancer patients is centrally located in the lobby of the hospital's main facility in Flushing. Printed materials are available in different languages from known sources including American Cancer Society and National Cancer Institute. Books from the hospital library as well as two desktop computers with internet access and a network printer will be available to patients and their families to print online resources. Cancer Care of Long Island - "Breast Cancer Support and Education Program" Free professional support services including outreach, counseling, education and information, financial assistance, practical help and referrals to other resources are provided to women affected by a breast cancer diagnosis. Materials are developed in Spanish and at appropriate literacy levels. Program Director: Patricia Spicer
Adelphi University - "Breast Cancer Educational Forum" This series of four breast cancer forums provides updated and innovative information related to breast cancer for individuals diagnosed with breast cancer, their families and the professional and lay communities. Forum topics include: "Ask The Experts - Medical Update and Enhancing Communication in Breast Cancer Treatment", "Coping with Anxiety and Depression - Techniques You Can Use To Feel Better", "Navigating the Health Care System", and a four-part series "Nourishing the Soul" which includes Intimacy After Breast Cancer, The One Who Laughs, Lasts, The Lost Art of Happiness, and Spirituality and Serious Illness. South Nassau Communities Hospital - "Breast Cancer Navigation Program" This program will conduct a patient navigation service to assist breast cancer patients at South Nassau Cancer Center from diagnosis through treatment and followup. Objectives of this program are to eliminate barriers to care such as transportation, child care, elder care, financial assistance and offers psychosocial support. Patients are given assistance with scheduling appointments, understanding their diagnosis, and given referrals to community-based services. Program Director: Dr. Christine Hodyl
St. Barnabas Hospital - "Mobile Mammography" Free mammography screening, a clinical breast exam, and education of self-breast examination are provided by the mammography van which serves the poor and uninsured communities of the Bronx. Program Director: Regina Lehman

Support is provided by phone, e-mail and one-on-one counseling through each person's journey from diagnosis, recovery and beyond. The customized information packets are designed to meet the needs of each individual caller.

Sid Jacobson JCC- "Strength to Strength" Program This wellness and aftercare program offers exercise instruction, group counseling, stress reduction workshops and nutritional counseling to women recovering from breast cancer as well as their caregivers. The program targets women who are in breast cancer treatment, approaching completion of treatment or have completed treatment and are looking to reduce fatigue and restore strength, flexibility and positive body image. Classes are offered free of charge at the Sid Jacobson Jewish Community Center in East Hills.

Cancer Care of Long Island - "Breast Cancer Support and Education Program"
Free, professional support services including education, information, referrals to additional sources of support, counseling and financial assistance is provided to women diagnosed with breast cancer on Long Island.

Hewlett House - "Women in Need of Support (WINS) Program"
Comprehensive cancer learning resource center providing psychosocial and educational services to women living with breast cancer. The home-like environment helps alleviate the social isolation, despair and hopelessness experienced by those touched by this disease. Group counseling, family support and medical research updates are offered.

Stony Brook University Medical Center - "Fit and Fabulous after Breast Surgery"
The Department of Physical and Occupational Therapy offers this program to women undergoing treatment for breast cancer. Softee camisoles are provided during the initial post-operative period after mastectomy for women who do not have the financial means to purchase one. Instruction and practice of therapeutic yoga is then taught to promote healing.

Monthly community health fairs provide materials in both English and Spanish covering topics such as the role genetics play in cancer, why screening is recommended, which women may be at higher risk for breast cancer, why early detection is important, and which programs are available to those without health insurance.

Created by a breast cancer survivor, this canvas tote bag is designed for the pre and post mastectomy patient. Items inside the bag include the "Women to Women Guide to Prepare for Mastectomy" and helpful supplies and personal items to prepare for surgery and aid in recovery. Bags are provided free of charge and are distributed to patients through physician's offices.

This program is designed to gather data specific to genetic risk with breast cancer patients and their daughters and create a training curriculum to establish clinical strategies for communicating this risk to women.

The South Fork Breast Health Coalition - "Wings of Hope"
An eight week Mind/Body program focused on overall health including yoga, nutrition and meditation which addresses critical aspects of emotional healing. It provides each individual with the coping mechanisms to deal with the fear, anxiety and stress of the disease.

American Cancer Society – “Transportation to Treatment For Breast Cancer Patients in Need”
Many elderly and underserved patients do not drive nor have access to a private vehicle; public transporation is not adequate; and many cannot afford the cost of a taxi service. The majority of uninsured breast cancer patients in Nasssau County receive treatment at Nassau University Medical Center. Door-to-door transportation is provided for any ambulatory breast cancer patient who demonstrates a need for transportation. Spanish-speaking cancer patients are offered the servivces of Taxi Latino, which provides Spanish-speaking taxi drivers.

Federation Employment and Guidance Service, Inc. (FEGS) – “Partners in Dignity Breast Cancer Education Project”
Educational forums for advance stage breast cancer patients, their caregivers and families provide them with tools and strategies for managing anxiety, depression and other emotional stressors. Women in the both the Jewish and general Long Island communities will be provided with services such as hospice and palliative care, symptom management and counseling.

This program provides culturally sensitive education and support to women of color in their communities focusing on information regarding breast cancer screening, early detection, treatment and support. Particular focus in on women in lower socioeconomic groups who are uninsured or under-insured on Long Island.
